Helen DeVos Children’s Hospital Appointment Policy
Due to Helen DeVos’s approval process and limited availability for dental surgeries you must comply with the following instructions:
- Update the surgery coordinator with any changes in dental or medical insurance as soon as possible. This ensures that all prior authorizations may be obtained in a timely manner.
- Schedule a pre-operative appointment within 30 days of the surgery date. The surgery coordinator will fax over the appointment request and required paperwork; however you are responsible for ensuring that the appointment is made.
- Pay the total copayment 1 week prior to the surgery date. The surgery coordinator will contact you at the week mark to collect this payment.
If after reasonable effort, the surgery coordinator is unable to reach you in regards to any of the above, they will cancel your child/children's surgery.
Outpatient surgery has limited availability due multiple factors that have been implemented by Spectrum Health. We understand that there may be times when unforeseen circumstances occur where you will need to cancel and/or reschedule the surgery. Depending on the nature of the cancellation, surgery may not be able to be rescheduled with our office or you may be charged a $250 re-appointment fee. The provider and surgery coordinator will review the chart and contact you with a final determination.
